I'm going to vote stork, because I feel like it, and because it fits a kid-friendly series and one where interspecies reproduction is plausible.

I say both. They could have been born naturally, then taken to a faraway hospital by a stork where all newborns are taken care of for a short time (pretty much like in reality, except it's another hospital instead of the one the baby is born - I guess it means there must not be many hospitals in the world) then taken back to their parents by the same stork.

I say both. Unlike the Zelda series that explicitly states that all the Links aren't the same. There is NO evidence whatsoever that Mario works in the same way. Besides, don't you think Myiamoto would have said something in the last 25 years?

Wrong within the context of the games for sure, but as fanon it makes some amount of sense.
Unless of course you count all the times that Pauline has appeared alongside Mario in the games after Donkey Kong, as well as the entirety of Donkey Kong '94. Then it becomes a jossed theory.
