Where do they come from? This is a summary of my theories so far.
All planets names are rather conjectural, and gotten from Mariowiki. You can view their pictures there as well.
SMG:
Good Egg Galaxy:
Planets:
1: Starting Planet: Likely from a small cluster of planetoids that were at some point inhabited. Well call it the 'Home Cluster'.
2: Boulder Planet: unknown
3. Flatland Planet: unknown
4. Bitten Apple Planet: A section of Giant Land used for growing food. Lets call it Giant Harvest.
5. Flip-Switch Planet: from a likely MASSIVE Flip-Switch Galaxy that was broken up by the arrival of Bowser. Lets call it the 'Flipswitch Cluster'.
6. Block Climb: unknown
7. Geo Planet: Possibly from a prehistoric-era cluster that broke off of the Mushroom World long ago. We'll call it 'Dino Cluster'.
8. Pear Planet: unknown
9. Rock Planet: unknown
10. Egg Planet: unknown
11. Tower Planet: a Hungry Luma
12. Pill Planet: an area with lots of gravity-changing things, possibly bordering the Home Cluster. We'll call it 'Gravity Cluster'.
13. Star 'Planet': artificial structure dedicated to the power of the Grand Stars.
14. and 15. Twin Tropic Planets: unknown
16. Water Planets: probably same area as Tropic Planets
17. Saucer Planet: unknown
18. Asteroid Planet: unknown
19. Magma Planet: unknown
Honeyhive: most of this was likely already there, as the bees state their neighbors are causing trouble again.
Loop-de-loop: artificially created using Magnetic Fields to keep the water in place. Must have a large gravity field below or Mario wouldn't have any trouble if he fell off a cliff.
Flipswitch Galaxy: remnants of the Flipswitch Cluster
Robot Reactor: completely artificial, however, a bending light issue causes a nearby organic planetoid to appear when looking at the first planetoid from a long distance away.
Space Junk Galaxy: completely as name implies, except for the Crystal Planet, likely from a cluster of similar planets, and the Sticky Planet, tarantox's lair. The scuttle bugs colonized nearby Stone planet and some of the space junk to guard Tarantox.
Battlerock Galaxy:
1. Saucer, Molecule, Missle, and Garbage planets: from Space Junk
2. Battlerock, Barrier, Metal Bowl, and Topman planets: space junk and large rocks converted into a fortress by the Topmen or a group of more humanoid robots. The laser drones (with the flat heads and beanie propellers) may be their creations.
Rolling Green: unknown, likely a grassland (duh)
Hurry-scurry: likely the center of a larger cluster of such platforms.
Bowser's Star Reactor: made of parts of various castles
Beach Bowl: mostly from a cluster of similar planets/planetoids (lets call it Beach Cluster), with one anomaly from a castle area, though the Glacier Planet came from a slightly cooler area.
Ghostly:
1. Starting planet and Mansion Planet: A ghost house and a walkway leading to it uplifted from Mushroom World. Several other sections were uplifted and became planetoids, leading up to the Bone Pen.
2. Cracked Wall and Bouldergeist planets: from a haunted arena in Mushroom World.
3. Bone Pen: unknown
4. Hollowed Out: unknown, mostly because black holes have nearly eaten them all away. Well call this the Hollowed Cluster. A planet from an area well call Trampoline Cluster is at the center of one of these.
5. Matter Splatter Mansion: from Matter Splatter Galaxy
6. Star bit shooter and Pull star planets: from Space Junk Galaxy
7. Spooky Speedster's race area: unknown, likely same place as Bone Pen
Bubble Breeze Galaxy: likely from a Poison Swamp
Bouy Base: from Beach Cluster, though fortified against invaders when the amphibious inhabitants left. The Poke Ball Planet is likely from Space Junk, though it seems that the technology was copied into the Laser Topmen, so it may have passed through the Battlerock at some point in time.
Bowser Jr.s Airship Armada: just airships, besides the Trampoline Planet, which may be from the Trampoline Cluster.
Gusty Garden: natural, except for the Apple Planets which come from the Giant Harvest and the Disk, ? and ! planets from the Space Junk Galaxy.
Freezeflame:
native area, besides the Freezy Peak Planet, which came from Cool Cool Mountain.
Dusty Dune: mostly from the area around Shifting Sand Land
Honeyclimb: from Honeyhive
Bowsers Dark Matter Plant: again, mostly from castle areas.
Gold Leaf: a twin area to Honeyhive, separated by unknown means and sent to its present location. The square planet, however, came from the Gusty Garden, and the Cataquack Planet may have, as well.
Sea Slide: mostly from the Beach Cluster, with a planetoid from the Hurry Scurry Galaxy.
Toy Time: from Space Junk with desserts from a very large refrigirator near the Giant Harvest.
Bonefin: from the Hollow Cluster, large enough that large amounts of rain and sediments created the planet in the center.
Bowser Jr.s Lava Reactor: from Melty Molten or Freezeflame. Freezeflame is more likely, as Prince Pikante, who is likely related to King Kaliente, is fought in an icy area.
Deep Dark:
1. Beach: from a haunted lake area?
2. Wooden: unknown
3. Gateway planet: from Home Cluster
4. Boo Box planet: space junk or toy time?
5. Cheep cheep planet: from Haunted Lake or Beach Cluster
6. Watermelon Planet: from Giant Harvest
Dreadnought: mostly space junk, though the Dreadnought itself would have been a travel ship or warship. Well say it came from an area called the Fleetyards.
Melty Molten: native area with a Geo planet from a Prehistoric Era, the fire and heat have mutated the dino piranha inside the egg.
Matter Splatter: native, see the Matter Splatter topic for a history.
Assuming hungry lumas simply become gravity fields that pull planets in to create galaxies, I'll also include hungry luma galaxies.
Sweet Sweet: same as dessert planets from Toy Time.
Sling Pod: space junk
Drip Drop: beach cluster
Bigmouth: Haunted Lake
Sand Spiral: from Dusty Dunes.
Boo's Boneyard: unknown, may be native, with the Bone Pen and Race Areas of the Ghostly Galaxy coming from here.
Snow Cap: unknown
Trial Galaxies: Rolling Gizmo: as Rolling Green
Loopdeswoop: as Loopdeloop
Bubble Blast: from Space Junk

Spin-Dig: mostly from a soft ground area, though the Purple Cylinder Planet may be from Space Junk.
Fluffy Bluff: NSMB world 7?
Flip-swap: unknown, likely an area near Hurry-Scurry and Flipswitch Cluster, as the technology seems similar.
Rightside Down: Home Cluster or Gravity Cluster?
Bowser Jr Fiery Flotilla: artificial. again.
