I'd like to do a little spiel on the fifth dimension here. It's simple if you can understand it.
There are three spatial dimensions: x, y, and z. X is forwards and backwards. Y is side to side. Z is height. And there's one temporal dimension: t. For every point in time there is a whole new set of x,y,z coordinates.
It so follows that, for every point in the fifth dimension w, there is a whole new set of x,y,z,t coordinates. This is the answer to the connection of worlds. Portals between worlds are wormholes that follow the fifth dimension's path, resulting in a new location and time at the other end.
Of course, some worlds are further away than others. Maybe the Mushroom Kindgom is a point 2.43 in the 5thD, and Flipside is at 2.71 with the paintings of SM64 at 2.29. And maybe Hyrule is way over at point 172.91, and the portal to it keeps changing its mind about what time period to deposit the traveler. (My theory on the Zelda timeline.)
As the number of dimensions you know increases, it becomes harder to travel them all. It's easy to walk forwards and backwards. A little harder to walk sideways. You can only walk up with a staircase or something. And time travel is not well understood, except for the fact that E. Gadd understands it and that everyone advances through time by default. Traveling through the 5thD would be nigh impossible if not for some supreme being, which may be the one who can call the heroes of the worlds together (Smash Bros).
